Many incidents and accidents have a long chain of risk factors leading up to them—called the Error/Accident Chain. This flight illustrates why it is critical to do AESOP at various points during the planning and preparation process, because the accumulating “sunk costs” make it much easier to postpone or cancel an operation in the early stages than at the end.
